Field Counselor, Military Family Clinic - Metrocare Services - Addison, TX
With a multidisciplinary team of psychiatrists, psychologists, licensed clinical social workers, licensed chemical dependency counselor, case managers, and veteran outreach workers, the clinic offers high-quality, evidence-based, and personalized outpatient mental health treatment tailored to meet the specific needs of veterans, particularly those from the Iraq and Afghanistan wars, and their families. The Military Family Clinic at Metrocare is part of the Cohen Veterans Network, a national nonprofit (501c3) network of clinics.
The Veterans Field Counselor Provides psychosocial/rehabilitative service to Veterans and their family members. Services may include, but are not limited to, assessment, development and measurable treatment goals and objectives, referral, linkage, advocacy, counseling, monitoring, crisis intervention and continuity of care. The overall goal of this position is to maximize the client’s potential level of functioning and increase access to care.
Responsibilities:
ESSENTIAL DUTIES AND RESPONSIBILITIES: (Other duties may be assigned.)
Provide home visits/ in field care including overall service coordination, referral, linkage, and advocacy to Veterans and family members with varying needs. Perform duties in concert with other members of an interdisciplinary treatment team. Complete psychosocial assessments on new clients. Develop measurable objectives and goals agreed upon by Veterans and family members. Actively assist in obtaining and modifying goals as needed. Document attempts to involve Veteran and/or family member, service providers, and caregivers (unless opposed by the client) in treatment process. Tracks and enters relevant data on a timely basis and maintains complete and organized client files. This includes electronic data tracking programs as well as case notes within client files. Review treatment plan as clinically indicated. Participate in interdisciplinary team staffing and coordinate treatment with team members to meet the needs of Veterans and their family members. Present psychosocial assessment findings. Update team on progress or lack of progress in reaching agreed upon goals. Provide relevant information that might affect treatment. Follow-up assigned Veterans and/or family members who miss a scheduled clinic appointment. Document attempts to contact. Report repeated unsuccessful efforts to contact to the treatment team. Follow-up within 24 hours on assigned clients who access emergency services. Report status-disposition to treatment team. Provide crisis intervention. Make home visits, work with police and other public servants as a needed to address crisis. Facilitate inpatient admission upon request of the treatment team. Provide continuity of services throughout inpatient stay. Participate actively in discharge planning. Meet with the client within 5 days of discharge into the community. Provide individual, family, and group rehabilitation sessions. Evaluate progress of session, solicit feedback from Veteran and or family member(s), and consult with colleagues and team leaders when dealing with unfamiliar/uncomfortable issues. Co-facilitate psychoeducational groups. Links to community resources for integration purposes, socialization, recreation, education, occupation and vocational needs of Veterans and family members who are served. Maintains records of pertinent program related information and compiles required data in reports. Prepares and submits reports following established timelines and procedures. Facilitates and mediates communication with Veterans and the Veterans Administration or other agencies. Responds to inquiries, phone calls, and e-mail in a timely and responsive manner.

Qualifications:
EDUCATION AND EXPERIENCE: Military experience preferred Bachelors Degree or Masters degree from an accredited college or university in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education, or early childhood intervention; or is a registered nurse Outstanding organizational, interpersonal and communication skills Outgoing, pleasant, professional manner; the ability to create and maintain contacts in the community; and clearly articulate mission and goals of the Military Family Clinic
REASONING ABILITY: Strong consultative abilities, conflict-resolution skills, and leadership abilities. Analyze problems, identify alternative solutions, project consequences of proposed actions, and implement recommendations in support of goals. Communicate clearly and concisely, both orally and in writing. Establish and maintain effective working relationships with those contacted in the course of work including diverse interest groups including advocacy groups and other governmental organizations.
COMPUTER SKILLS: Intermediate level on MS Excel, Email and word processing programs Ability to utilize Internet for resources.
