Hollywood Veterans Center is a residential facility for veterans of the Afghanistan and Iraq wars that are experiencing severe levels of difficulty readjusting to civilian life. Residents have the goal of obtaining permanent housing, financial security, and a personal support system. What makes the program unique is individualized approach and focus on compassion and tolerance: we will emphasize service that builds personal growth, self-determination, and community reinforcement; we will take measures to ensure family preservation and/or reunification; and we will recognize and celebrate the achievements of the individual.


Responsibilities:
As a key member of The Hollywood Veterans’ Center, the Clinical Case Manager provides direct clinical services to Veterans who are participating in the program who have histories of serious mental illnesses and/or substance abuse.


Under supervision of licensed Program Manager, the Clinical Case Manager:


  • Assesses, diagnoses, and treats dually diagnosed Veterans using multiple approaches in both individual and group settings

  • Works with participants and families that are experiencing a variety of psychiatric, medical and social problems by utilizing individual, group, and family counseling skills

  • Conducts ongoing psychosocial assessments to determine needs of Veterans and family members and encourages active participation in the treatment planning process

  • Formulates and implements treatment plans identifying individual’s goals, strengths, coping skills and assistance needed

  • Participates with VOALA and VA agencies in discharge planning; ensures that discharge plans are executed in timely and appropriate manner

  • Follows-up after discharge to support long-term placement success

Requirements:

  • Bachelor’s degree in social work, psychology, or counseling from fully-accredited college or university; Master’s degree (MSW) preferred

  • At least 2 years of experience in mental health/substance use disorder treatment and/or working with homelessness

  • Effective written and oral communications skills with people from varied backgrounds

  • Proven working knowledge of Microsoft Office; ability to learn and use program-specific software

  • Valid California driver’s license and access to dependable transportation with adequate insurance

Additional Requirements:

  • Coursework in substance use disorder counseling a plus

  • U.S. Armed Forces Veteran or experience working with Veterans preferred

  • At least 1 year in residential treatment setting preferred

  • Basic knowledge of psychosocial treatment modalities, especially cognitive behavioral therapy (CBT), dialectical behavior therapy skills training (DBT), motivational interviewing (MI), relapse prevention (RP), Seeking Safety preferred

  • If recovering substance user, must have at least 3 years of sobriety
